Masthope Lineage Pass Assignments

Please note: Passes can take up to 3 weeks to be processed.

Add the names, relationships and photos for your family lineage passes in the form below. Only add information for lineage family members. For example, if you have 5 lineage passes, you will receive 7 guest passes. For this particular example, fill out information for 5 of 12 passholders in the form below… leave the remaining 7 blank so they can be issued as guest passes.

• If you have already submitted a paper/PDF copy of the Lineage Pass assignment form, you do not need to fill this form out again… we will process your paper copy. For those owners who do not have a photo uploaded to the new system, we will transfer photos from the old system. If you believe your photo is outdated or would like a new one on file, please send us a photo with your name by emailing info@masthope.org. IF YOU ALREADY HAVE GREEN RFID CARDS FROM A PRIOR SKI SEASON, you have a photo uploaded in the system. We will be reviewing all photos to ensure accuracy and clarity.

• If you have not submitted a paper/PDF copy of the Lineage Pass assignment form, please fill the following form out completely. Please make sure to include birth dates for each person. IF YOU ALREADY HAVE GREEN RFID CARDS FROM A PRIOR SKI SEASON, YOU DO NOT NEED TO UPLOAD A PHOTO, but we ask that you fill out other form fields forms completely (Name, Relation, DOB) so we can verify each passholder’s information. If some people in your family have green RFID cards and others do not, make sure there is a photo for every person who does not yet have a pass. All blank entries will be given as guest passes.

SUBMIT AN UPDATED DEED: You must provide us with an updated deed in addition to the information requested below. Please mail the office a copy or email a digital copy to info@masthope.org. Passes will not be issued without an updated deed. If you already submitted a copy of your deed with your paper/PDF copy of the Lineage Pass assignment form, you do not need to resubmit another copy when resubmitting this form.

PASS PHOTO REQUIREMENTS: All photos must be a headshot taken within the last 2 years to reflect your current appearance. Your full face must be visible. If you normally wear prescription glasses, a hearing device or similar articles, they may be worn for your photo, however sunglasses or non-prescription glasses with tinted lenses are not acceptable. Passes will not be issued without an appropriate photo.

PASS POLICY – HELPFUL INFO: Each Masthope property will receive a maximum allocation of 12 ID passes. ID passes can be assigned to persons of the Property Owners’ lineage up to and including the maximum allocation. For lots with multiple Property Owners, the allocation can be split between the two owners at the discretion of the owners. The allocation of these passes is up to the Property Owner. Any ID pass not assigned as lineage will be given as a guest pass. Immediate lineage to the Property Owner includes: Spouse, Children, Grandchildren, Great Grandchildren, Parent, Grandparent, and Great Grandparent.
